The body of a refused start, on any channel.
detail is a fixed sentence. code names the closed reason when the
refusal has one: a phone start's missing person, a fax start's channel,
driver or identity, or a document that cannot be a fax. It is absent for a
start refused by a plain validation failure, so a client must treat it as
optional.
