One workspace the endpoint covers, and its Form opt-in.
formOptIn is not_applicable when the endpoint does not subscribe
to data_record.created or data_record.updated. It is
covered_but_opted_out when those events are subscribed and no Form
has webhookEventsEnabled. opted_in means at least one Form does.
