First, the Form that holds their record.

## Create the Care coordination Form

In Knowledge, create a Care coordination Form. Add Coordination status as an Enum: Open, Needs follow-up or Resolved. It's Admin Only, so only staff set it.

## Acknowledgment

Now the Journey itself, starting with the acknowledgment.

## Create the Journey

In Journeys, create a V2 Journey and name its first step.

## Start with an open request

Auto Enroll: members whose Coordination status is Open join automatically.

## Acknowledge receipt

Add a Send Email action saying we got their request.

## Inform coordinator

Next, telling the coordinator.

## Assign the open request

Then a notification, so the Care Coordinator hears right away.

## Next day: follow up

Next, the check a day later.

## Check the status later

Add a Check request status step that waits one day.

## Create two follow-ups

Then one step for each route: follow up, or check the status.

## Route follow-up status

This route checks the Form. Only Needs follow-up goes this way.

## Remind the coordinator

The coordinator gets a reminder to take it on.

## Update the member

The patient hears their next step is being reviewed.

## Ask coordinator to check

And if it's still Open, or has no status.

## Verify uncertain status

Otherwise, the coordinator is asked to check its status. No patient email.

## Stop when Resolved

Finally, when to stop.

## End when resolved

Add an End Condition: Resolved ends the Journey.
