First, the Form that holds their record.

## Create the Heart health check-ins Form

In Knowledge, create a Heart health check-ins Form. Add Callback requested as a Boolean: yes or no.

## Callback offer

Now the Journey itself, starting with the callback offer.

## Create the Journey

In Journeys, create a V2 Journey and name its first step.

## Auto enroll the program

Auto Enroll: members with the Enrolled label join automatically.

## Ask about a callback

Add a Send Email action asking if they'd like a callback.

## Wait for answer

Next, waiting for their answer.

## Wait for the recorded answer

Add a step that waits until Callback requested is recorded.

## Separate the answers

Then add one step for each answer.

## Route yes and no

Yes goes one way, No the other. With no answer yet, it keeps waiting.

## Yes: inform coordinator

Next, when they want a callback.

## Alert the coordinator

On the Yes step, the Heart Health Coordinator gets a notification to arrange contact.

## Acknowledge a request

Their email says a coordinator will review the request.

## No: short note

And when they don't.

## Acknowledge the other answer

No gets a short thank-you.

## Stop once answered

Finally, when to stop.

## Reach the End

Both answers then reach the End.
